You don't need a polling card to vote - you might need Clare to appoint you as your proxy unless you both want to make the trip to your old polling station. Hackney (?) council electoral services should be able to help.

I used to do Clerkenwell-Cannon Street and it's pretty easy - 521 bus from Chancery Lane tube/ Holborn Viaduct, or failing that, I think a 17 does it too.It's walkable as well, and Moorgate station is only 10 mins (brisk!) walk from Cannon Stret.Fail...

It is a train bridge - the old North London Line to Broad Street, which will be reused when the East London Line is (eventually) extended.
